Join us for an insightful webinar on Reimagining Advance Care Planning (ACP) Through an Accessible Lens on December 3, 2024, at 10:00 AM. With Canada’s aging population and rising disability rates, accessible ACP tools are more critical than ever. This presentation will highlight Advance Care Planning Canada's new project, Making Advance Care Planning More Accessible for Persons with Disabilities, focusing on intersectionality and universal design. 

Presenters Karine Diedrich and Merry Parkinson will discuss project outcomes, including the 2024 National ACP Public Poll, an updated ACP guide, and new approaches to integrating supported decision-making.
